Application Process

STEP 1

  • Potential Fellowship Candidates are sent an application form and are informed about basic financial and legal requirements. Fellows must have one of the following:
    • Be a Canadian citizen and be eligible for CPSO licensing in order to have a self-funded fellowship
    • If not a Canadian citizen, the candidate must be funded by an officially recognized source - For example, such as a state, university, or scholarship that covers all expenses as required by the Faculty of Medicine
    • If not a Canadian citizen, applicants are also informed of the need to apply for a study visa.
  • Fellows are to contact the CPSO and CMPA to become informed of their requirements:
    • The College of Physicians and Surgeons of Ontario, 80 College St, Toronto, ON M5G 2E2 (416) 967-2600 or 1-800-268-7096 Ext. 221
    • The Canadian Medical Protective Association, Membership Services. P.O. Box 8225 Stn T Ottawa, ON K1G 3H7 (613) 725-2000 or 1-800-267-6522

STEP 2

  • As soon as it is determined that the applicant may meet basic requirements, he or she is directed to a potential supervisor. If there is general agreement on a possible fellowship, the candidate proceeds with completion of the application form and arrangements are made for a meeting between the supervisor and candidate. This meeting is intended to more formally determine the feasibility and nature of the fellowship, including goals and objectives.

STEP 3

The supervisor is sent Form D (outlining the basic description of the fellowship) to complete and have signed at the time of a meeting with the candidate.

  • If the supervisor agrees to the Fellowship, he or she must contact hospital administration to ensure that the Impact Analysis is done and that the application process for hospital privileges is initiated.
  • If the Fellow is from abroad he or she may be required to do the Pre-Entry Assessment Program (PEAP) which requires a language assessment period of a minimum of 3 weeks to a maximum of 12 weeks to ensure that the fellow is competent to undertake his or her responsibilities.

STEP 4

The completed application form, including reference letters, the statement of goals (on letterhead signed by both supervisor and applicant) and all other listed documents are mailed or delivered to Dr. Greg Motayne. Once the file meets all Faculty requirements, Dr. Motayne presents the file to the PGE committee for approval.

STEP 5

The Director of Education provides the final letter of acceptance and the file is sent to the Faculty of Education by Lynn Bertrand.

STEP 6

If the Fellow is not a Canadian Citizen and is coming from abroad to do a fellowship, he or she is required to do the PEAP before the CPSO grants the formal Educational license.
